We have chosen to discuss data collection forms better suited for therapy with preschool children, small group therapy, and culturally and linguistically diverse children. It is also important to think about specializing your data forms to better suit other specific therapy situations. They feel it is especially valuable to keep good data, so that information can then be collected about best outcome measures that would inform the practices of speech language pathology in the school. The American Speech-Language-Hearing Association understands the importance of data collection but also how it can be especially complex in the school setting, stating that “the number, variations, and combinations of modes of delivering services are greater in the school setting than in other settings” ( Gallagher, Swigert, & Baum, 1998 ). Data collection is important even more so in the schools today, as documenting the progress of the child is important for Medicaid purposes and making sure IEP goals are met.

As the saying goes “if it isn’t documented, then it didn’t happen”.
